Player Release Form

A player is released immediately from a team when the player and the coach/manager signs the form below.  If not, a player will be released 7 calendar days after the ISA State Director or the ISA Fast Pitch State Director signs the form below.  A player may be released only 2 times during the normal qualifying seasons leading up to the World Series. 
A copy of this form should be maintained at all times.  For youth players, maintain a copy with a copy of your birth certificate and give to your current coach/team manager.
